Volunteer Buyout $65 *

As our needs for the club grow exponentially so does our need for help as we attempt to keep costs low. a) Commit to faithfully volunteer across a select list of options shown below OR b) Pay a small $65 volunteer buyout and simply just arrive (we use these funds to hire out certain activities that can't be covered by existing volunteers). Participation and Liability Waiver *

Trident Football Club Medical Care, Consent, & Release Form: I certify that my child is in good physical health and has my permission to participate in all activities associated with the Soccer club. I authorize the coaches of the club to act in their best judgment in any emergency requiring medical attention. I authorize all medical, surgical, diagnostic, and hospital procedures as may be performed or prescribed by a treating physician for my child if I am unable to be reached in an emergency. I understand that any facilities used for hosting, all soccer coaches and staff, nor anyone else connected with the club assumes any responsibility for accidents (medical or dental) or other injuries incurred as a result of attendance during the season. I will furnish insurance for my child.
